Site Supervision Contract
This contract serves as verification and as a description of counseling supervision provided by the above-named Site Supervisor to the above-named Walden University School of Counseling Student.
Purpose, Goals, and Objectives:
- Monitor and ensure the protection and welfare of clients seen by the Supervisee.
- Promote the development of the Supervisee’s professional counselor identity.
- Assess and provide ongoing feedback of skills and dispositions to ensure developing competency.

Meetings and Communication
The Supervisee agrees to meet with the Site Supervisor as mutually arranged and for a minimum of one (1) continuous hour of live, in-person or videoconference individual or triadic (a maximum of one additional student) supervision each week of the quarter. Supervision will at a minimum entail review of student work, review of student cases, and feedback to further the development of student skills, professionalism, professional identity, and conceptualization.

Terms of the Contract:
This contract will commence upon approval of the field experience application and enrollment in field experience and will continue until the termination of the field experience. This contract is subject to revision at any time by the Supervisor, or the Supervisee with approval of the Supervisor. We agree to the best of our ability, to uphold the directives specified in this Supervision Contract and to conduct our professional behavior according to the ethical principles of our professional associations.
Field Experience Faculty Group Supervision Contract
This Faculty Supervision Contract is to be completed by the Faculty Supervisor (Course Instructor) and Student.
Students cannot participate in supervision until this contract has been signed by all parties and submitted in the field experience classroom.
Introduction:
This contract serves as verification and as a description of counseling supervision provided by [Instructor] to the following Walden University School of Counseling Students.
Purpose, Goals, and Objectives:
- Monitor and ensure the protection of the welfare of clients seen by the Supervisee.
- Promote the development of the Supervisee's professional counselor identity and competence.
- Fulfill requirements in preparation for the Supervisee's pursuit of the completion of the field experience requirement for the academic program described above.
- Serve as an assessor of Supervisee skills and dispositions.
- Serve as a Gatekeeper to the profession in accordance with the Council for the Accreditation of Counseling and Related Programs expectations and American Counseling Association's Code of Ethics and Standards of Practice.
Context of Services:
- Mentoring, skills training, and support of the development and integration of student's theoretical orientation.
- Minimum of two hours of group supervision each week for at least 9 out of 10 weeks of the quarter.
- Completion of all required documentation and student evaluations.
Method of Evaluation:
- Feedback will be provided by the Supervisor during each meeting.
- Specific feedback provided by the Supervisor will focus on the Supervisee's demonstrated counseling skills, case conceptualization, treatment plan and overall professional development.
- Supervisor will document meetings and Supervisee can request a copy of written documentation of each meeting.
- Supervisor will review the Meditrek Mid-Quarter Evaluation, recordings/alternative assignments, and complete the Meditrek Supervisor Evaluation of Field Experience Student and seek additional remediation or support for the student if warranted.
- Supervisor will sign all appropriate field experience documents (e.g. Time Log, evaluations, etc.).
Duties and Responsibilities of Supervisor and Supervisee:
Supervisor:
- Examine clients' presenting issues and treatment plans.
- Listen and provide feedback on the audio/video recordings of the Supervisee's counseling sessions or alternative assignments. Two (2) are required for each term of field experience.
- Collaborate with Supervisee's Site Supervisor to support the student, including conducting a virtual site visit to assess Supervisee's performance.
- Assess and sign final documents (Time Log, etc.) at the end of the term.
- Encourage Supervisee to learn, discuss, and apply developmental theories, counseling theories, clinical skills, best practices, counseling techniques.
- Encourage the integration and development of a theoretical orientation approach in compliance with the American Counseling Association's Code of Ethics and Standards of Practice.
- Monitor Supervisee's basic and advanced counseling skills.
- Intervene if it is discovered that client welfare may be at risk in collaboration with the Supervisee's Site Supervisor.
- Ensure the American Counseling Association's Code of Ethics and Standards of Practice are upheld.
- Ensure that the Health Insurance Portability and Accountability Act (HIPAA) is upheld.
- Ensure that the policies outlined in the Field Experience Manual are upheld.
Supervisee:
- Uphold the American Counseling Association's Code of Ethics and Practice Standards.
- Uphold the policies outlined in the Field Experience Manual.
- Uphold the School of Counseling's Professional Dispositions, including: Engagement, Accountability, Relationships, Sensitivity, Impartiality, Discipline, Awareness, Growth, Communication, and Congruence.
- Be prepared to obtain and preview counseling session recordings for review or process the alternative assignments during supervision meetings.
- Work collaboratively to navigate supervision by both Site Supervisor and Faculty Supervisor.
- Interact with the Doctoral Student Co-supervisor in accordance with the directives of the Faculty Supervisor, if applicable.
- Be prepared to discuss all client cases, as well as professional and personal development while enrolled in field experience.
- Discuss client case conceptualization made and approach and techniques used.
- Discuss your working client case conceptualization and the progress of your approach and techniques in a collaborative spirit, constantly seeking to improve and enhance your effectiveness with each client and family.
- Implement supervisory directives in subsequent sessions and discuss the directives or results in subsequent supervision meetings.
- Consult with Supervisor in cases of emergency.
- Inform Supervisor of any of the following occurrences immediately after they occur:
- Incidents of restraint
- Incidents of violence to clients and/or clinician
- Incidents of violence to all others
- Disclosed thought of client regarding violence to others
- Knowledge of any suicidal thoughts or intent of client
- Any possible confusion on, or breach of, appropriate boundaries
- Any known violations of confidentiality and/or clients rights
- Reports of abuse or neglect to Child Protective Services (CPS)
- Attend all scheduled group supervision calls (one session may be missed in the event of an emergency).
- Be fully present and engaged for the entirety of the weekly supervision call. That includes:
- Video and Audio
- Not engaging in distracting behaviors
- No multitasking
- No driving

This Individual/Triadic Supervision Contract is to be completed by the Individual/Triadic Supervisor and Student.

This contract serves as verification and as a description of counseling supervision provided by [Triadic Supervisor] to [Student] a Walden University School of Counseling Student.
Purpose, Goals, and Objectives:
- Monitor and ensure the protection of the welfare of clients seen by the Supervisee, if applicable.
- Monitor and support counselors in training who receive supervision by Supervisee, if applicable.
- Monitor and support student teaching of Supervisee, if applicable.
- Monitor and support Supervisee's research, if applicable.
- Monitor and support Supervisee's leadership and advocacy activities, if applicable.
- Promote the development of the Supervisee's competence as a counselor educator in training.
- Fulfill requirements in preperation for the Supervisee's pursuit of the completion of the field experience requirement for the academic program described above.
- Serve as an assessor of Supervisee skills and dispositions.
- Serve as a Gatekeeper to the profession in accordance with the Council for the Accreditation of Counseling and Related Programs expectations and American Counseling Association's Code of Ethics and Standards of Practice.
Context of Services:
- Support the student's development of appropriate theory, skill training, and counselor educator identity development within each selected internship domain.
- Minimum of one hour of individual or triadic supervision each week.
- Completion of all required documentation and student evaluations.
Method of Evaluation:
- Feedback will be provided by the Supervisor during each meeting.
- Specific feedback provided by the Supervisor will focus on the Supervisee's demonstrated skills in the domains in which he/she is completing intership (e.g. counseling and clinical documentation skills for counseling experiences) and overall professional development.
- Supervisor will document meetings and Supervisee can request a copy of written documentation of each meeting.
- Supervisor will sign all appropriate field experience documents (e.g. Time Log, evaluations, etc.).
Duties and Responsibilities of Supervisor and Supervisee:
Supervisor:

- Assess and sign final documents (Time Log, etc.) at the end of the term.
- Supervise and assist the Supervisee to develop appropriate theoretical, skill, and practice standards appropriate to each of the Supervisee's internship domains.
- Intervene if it is discovered that the welfare of the Supervisee or his/her client(s), supervisee(s), and/or student(s) may be at risk.
- Ensure the American Counseling Association's Code of Ethics and Standards of Practice are upheld.
- Ensure that the Health Insurance Portability and Accountability Act (HIPAA) is upheld.
- Ensure that the policies outlined in the Field Experience Manual are upheld.
Supervisee:

- Be prepared to discuss all supervisees and supervision files, as applicable.
- Be prepared to discuss classroom activities, lesson plans and student development, as applicable.
- Be prepared to discuss all domain-specific work samples.
- Implement supervisory directives in subsequent sessions and discuss the directives or results in subsequent supervision meetings.
- Consult with Supervisor in cases of emergency.
- Inform Supervisor of any of the following occurrences immediately after they occur:
