How do I complete an application for field experience?

Where can I find my Meditrek login credentials?

You should have received this information via email during your first quarter of enrollment in the program. If you have misplaced your Meditrek login credentials, email for assistance.

What is the application deadline?

What do I need to complete before the application submission window closes?

You will need to submit your application with all of the items on the initial checklist in Meditrek (Phase 1) completed on or before the day the application submission window closes. The Phase 1 application checklist includes the following items:

  1. Student Information Form
  2. FERPA Release Form
  3. Field Site Information Form
  4. Site Supervisor(s) Resume(s) or Work Experience Summary(ies)

Additional items will be added to your checklist after the SOC-OFE and site supervisor reviews of your application are complete. These items need to be completed no later than 2 weeks prior to the start of your field experience. See the Application Process section of this website for details:

My supervisor is having trouble logging into Meditrek. How can I help?

Provide the following Meditrek Login Troubleshooting information:

  1. Try a different web browser (we recommend Google Chrome or Firefox).
  2. Type login information instead of copying and pasting it.
  3. Ensure that you have typed (with the “edu” in the front) in your browser’s address bar
  4. Email for further assistance

What does my site supervisor need to complete on my Meditrek application?

You complete the initial submission of your application in Meditrek (your site supervisor does not access Meditrek until after the initial SOC-OFE review of your application is complete). You will, however, need to gather information from your site supervisor in order to complete your application.

  1. Complete as much of the Meditrek application as you can on your own.
  2. Bring questions you cannot answer to your site supervisor (either schedule a meeting with them to complete it online together or email/call them with the specific information that you need to complete your application, such as contact information, license number, degree information, and their resume/CV.

After your application is submitted and the SOC-OFE completes the initial review, your site supervisor will receive Meditrek login credentials, and will need to complete the following in Meditrek:

  1. Review of your application
    • Your site supervisor will need to review the information submitted and accept the application, or make revisions, should they be required.
  2. Site Supervisor Learning Agreement
    • This simple online form will be available in Meditrek after your site supervisor accepts your application.

I was not able to submit my application by the deadline. What do I do?

Late applications are not accepted, so you will need to postpone the field experience and resubmit your application within the appropriate application submission window.

If you have extenuating circumstances that you believe warrant an exception to this policy, you can submit a Field Experience Petition to request an extension. You will need to have a site secured in order to complete a petition.

How do I obtain a professional liability insurance certificate?

Students need to secure their own liability insurance for field experience. See the following links for details:

Why can't I access the Site Supervisor Learning Agreement?

How do I close a Meditrek application?

How do I make changes to an application that has already been submitted?

Does my TA Mentor need to sign my TA Contract and my Meditrek application?
