Field Experience Application Steps

Step 1: Identify your Field Experience start date by contacting Student Success Advising. It is important to know when you are scheduled to begin your field education courses. You'll be asked to choose a term start on your field application.

Step 2: Apply for Field in Meditrek.

General Instructions for MSW and BSW students

  • To submit your application packet, you must submit: application, resume, certificate of insurance, and Home Visit Waiver Form. All parts must be complete before your application can be reviewed.

  • It is recommended to upload and assign your documents first, then complete your application form. ​

Step 3: If all parts of the packet are correct, your packet will be accepted. Watch for your application approval email and will be assigned a Field Experience Coordinator to assist with your placement process. 

Applications may be returned for the following reasons:

Academic Readiness - If we can't confirm you have met all of the academic requirements to be ready to begin your field placement, we will return your application with a status of "pending readiness" and re-review it once we can confirm your readiness.

Missing Resume - You are missing a required document for your application, we will return your application to you.  You will have 3 days from the application deadline or the date of your returned application (whichever is later) to submit your documents. Once you have resubmitted them, we will review the application within 3 days.

Proof of Insurance Coverage - If you do not provide the proper proof of insurance, we will return your application.  You will have 3 days from the application due date or the date of your returned application (whichever is later) to submit your proof of insurance.  Once you have resubmitted the proof, we'll re-review the application within 3 days. 

Your application packet is NOT complete if you do not click the "submit" button. 

*A Note About Deadlines:

  • Application Deadline: This is the date your four-part application packet is due in Meditrek. You can submit your packet earlier than this date; however, the review may be delayed if you’re submitting your application early.
    • We usually begin processing next quarter packets at the previous quarters Lead Agency Form deadline.
    • You’ll be assigned a Field Experience Coordinator after your application packet is accepted.
  • Lead Agency Form Deadline: This is the date a prospective agency should be submitted in Meditrek by. The internship doesn’t need to be secured by then, but you should contact the agency prior to submitting their information.
  • Field Placement Deadline: This is the final deadline to have an approved internship. Your Field Experience Coordinator partners with you to ensure sites can provide an appropriate Field Experience as outlined by the CSWE guidelines. Site paperwork and task conversations must be completed prior to this date.

What Will I Need to Apply

We'll need the following information:

  • Application Form
    • Identifying information such as your Student ID, address, contact information, etc.
    • Emergency contact information
  • Home Visit Waiver Form
    • Requires initials and password.
  • Current resume
    • Professional, appropriate to share with agencies
  • A certificate of professional liability insurance (which will denote the coverage amount and dates of coverage).
    • You are required to purchase professional liability insurance to cover both quarters of your Field Experience. Coverage is valid for one year from purchase date.
      • You may purchase the insurance from whichever company you choose. NASW offers liability insurance at a student rate when you become a member (http://www.naswassurance.org/pli/professionals/). You may also contact American Professional Agency (http://www.americanprofessional.com).
      • For students residing in Canada, CASW Liability Insurance offers insurance at a student rate when you become a member (https://www.casw.bmsgroup.com/home.html)
      • Students are required to maintain liability insurance with a single limit of no less than one million dollars ($1,000,000) per occurrence and three million dollars ($3,000,000) annual aggregate.
      • Note: Worksite insurance (any type of insurance provided by an employer) does not qualify for liability insurance. All students are required to purchase professional liability insurance, as outlined above.

Please note the following pre-requisite courses that must be completed prior to beginning Field Experience:

BSW: All Courses AND Skills Lab

Generalist (Foundation): SOCW 6101, SOCW 6200, SOCW 6210, and Skills Lab 1

Advanced Practice: SOCW 6510, SOCW 6060, SOCW 6090, and Skills Lab 2 (Concentration is usually the final two quarters of the program.)

What's Next, After My Application is Approved?

The Field Office will review your application materials. If additional information is required, you will be notified via email. If your packet is complete, it will be approved.  You will receive an approval email with instructions for submitting the name of your agency of choice.  

You'll submit your agency of choice in Meditrek using the Lead Agency Form during week 5 of your Skills Lab course.  

The Lead Agency Form allows you to enter the name of the agency where you are interested in interning.  You are required to provide the following:  Agency Name, Contact Person, Phone Number, and Email Address.

After our office receives your completed Lead Agency Form, you'll be assigned a Field Experience Coordinator who will reach out to you and your agency of choice.
